29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that will prevent pull from being directly transmitted to joints or terminal screws.  (a)  On or about September 30, 2022, Building 4, Station 2: Employees were conducting welding operations in close proximity to a 208-volt, 60-amp, 3-Phase welding unit (4 pack) cable/plug assembly with exposed internal conductors.  No abatement certification or documentation required for this item.

29 CFR  1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2), unless the nature of the work in progress or the physical conditions prohibit the use or installation of such guardrails.  (a)  On or about September 30, 2022, Main Deck: Employees were welding the bulk head wall on the main deck without fall protection approximately 16.5 feet above the lower level.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
