29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:  a) 901 Market Street Wilmington, DE 19801: Employees worked without appropriate personal protective equipment to protect their skin [e.g. hands] while performing finishing work. The employees were exposure to caustic chemicals in the form of a wet cementous material  containing  Coal Ash, Portland Cement, Crystalline Silica and Quartz while applied to the wall tiles within the unfinished apartment bathrooms with their bare hands as observed on or about October 3, 2022.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) 901 Market Street Wilmington, DE 19081:  Employees were not provided effective training and were exposed to hazardous chemicals, to include, but not limited to Portland Cement while performing tile setting activities when applying wet concrete as documented on or about October 3, 2022.
