29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  On or about October 5, 2022, employees were exposed to a struck-by hazard while working in close proximity to a truck mounted auger drill.

29 CFR  1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers.  On or about October 5, 2022, employees were exposed to a fall hazard while working in close proximity to a hole that was being drilled without having a fall protection system in place.
