29 CFR  1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.   On or about October 5, 2022, the employer did not ensure the employee using a DeWalt cut off saw to cut masonry material was protected from being exposed to silica with the use of engineering controls, work practices, and respiratory protection.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:   On or about October 5, 2022, employees performing masonry work utilized an extension cord to energize a DeWalt cutting saw with the outer sheathing split exposing internal wiring. The employees were exposed to electrical shock hazards.
