29 CFR 1926.102(a)(1): Eye and face protective equipment being used did not used meet the requirements specified in Z 87.1-1968:    The employer was failing to protect employees from struck by hazards. This was most recently documented on October 7, 2022 at a residential construction jobsite located at 1204 19th Street, Beatrice, Nebraska. Employees were operating pneumatic nail guns while shingling a roof without the use of eye protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling:  The employer was failing to protect employees from fall hazards. This was most recently documented on October 7, 2022 at a residential construction jobsite located at 1204 19th Street, Beatrice, Nebraska. The employees where installing shingles while working at a height of approximately 9 feet 5 inches measured from the ground to eave without fall protection.

29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer was failing to protect employees from fall hazards. This was most recently documented on October 7, 2022 at a residential construction jobsite located at 1204 19th Street, Beatrice, Nebraska.  Employees were using a ladder to access the roof that was not extended over the upper landing surface at least three feet.
