29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about October 7, 2022, the employer did not ensure a competent person was conducting frequent and regular inspections of the job site to ensure fall protection was being worn, fall protection systems were installed properly, ladders were properly maintained, and job site conditions were maintained safe.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about October 7, 2022, employees were operating pneumatic nail guns without eye  protection while installing shingles, exposing employees to  eye injury.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about October 7, 2022, employees conducting roofing work on a 6:12 pitch roof on the South side of the structure, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet at the eave and Twenty-five feet at the ridge.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about October 7, 2022, the employer did not insure employees had received  fall protection training for employee(s) exposed to fall hazards while working on a 6:12 pitch residential roof without a fall protection system installed, to recognize conditions associated with this type of fall hazard, and to give employees the knowledge needed to remove the hazard and install a correct fall protection system.
