On or about October 7, 2022, at the above addressed jobsite, an employee performing exterior ornamental steel installation from a Genie Telescopic Lift Model #S-80 Serial #S8008-6984, approximately eighteen (18') feet above the ground, was wearing a full body harness without a lanyard attached to the employee, thereby exposing the employee to fall hazards.
