Two employees were uninstalling exterior security cameras at the workplace. An employee had lifted another employee utilizing the forks of a forklift to reach the exterior security camera. The employee was exposed to a 16-foot fall hazard to the concrete ground below.

At the workplace located at 4400 Northwest 41st Street Suite 200, Riverside, Missouri 64150, an employee was exposed to a serious fall hazard. Two employees were uninstalling exterior security cameras at the workplace. An employee had lifted another employee utilizing the forks of a forklift to reach the exterior security camera. The employee was exposed to a 16-foot fall hazard to the concrete ground below.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.   At the workplace located at 4400 Northwest 41st Street Suite 200, Riverside, Missouri 64150, Employees were exposed to a serious fall  and struck-by hazards. The employer did not ensure that an employee who was operating a Toyota LP forklift model number 8FGCU25 was competent to operate the powered industrial truck safely, as demonstrated by the successful completion of a training and evaluation program. The employee operating the forklift was observed lifting another employee standing on a wooden pallet with the forks of the forklift.
