On or about October 5, 2022, and at times prior thereto, employees working at heights and exposed to fall hazards were not provided training to recognize and eliminate fall hazards and how to utilize fall protection safely exposing the employees to fall hazards.

29 CFR  1910.140(c)(13)(ii): Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two.  On or about October 5, 2022, and at times prior thereto, where employees perform tree trimming activities from heights in trees, the employer did not ensure anchor points, such as but not limited to the entire tree and the base/roots as an anchor, were designed, and used under the supervision of a qualified person exposing the employee to fall hazards.
