29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a. At the Dock area, located at 12807 San Jose Boulevard in Jacksonville, Florida 32223: On or about October 12, 2022, the employer exposed two employees to fall hazards ranging from 6 to 30-foot, in that fall protection, such as, but not limited to, safety harnesses with lanyard attached to the boom were not utilized by the employees, while installing a light bulb.
