29 CFR  1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Jobsite, 57 East Main Street, Uniontown, PA 15401:  On or about October 12, 2022, the working level of scaffolds on the south and east sides of the building were not fully planked.  Employees working on the scaffolds were exposed to a fall hazard of approximately 16 feet to the ground below.

29 CFR  1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a) Jobsite, 57 East Main Street, Uniontown, PA 15401:  On or about October 12, 2022, guardrail systems were not installed along all open sides and ends of platforms on scaffolds on the south and east sides of the building.  Employees working on the scaffolds were exposed to a fall hazard of approximately 16 feet to the ground below.
