29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about October 12, 2022, employees were framing a home under construction without hard hats on while work was occurring directly above them over head, exposing employees to struck by hazards such as falling debris.

29 CFR  1926.102(a)(1): The employer did not  ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about October 12, 2022 , employees were installing shingles on a  residential home under construction using a pneumatic nail gun with no eye protection, exposing employees to struck by hazards from flying objects or debris.

29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:  On or about October 12, 2022, employees were securing shingles to the roof of a new residential home under construction without the use of fall protection, exposing the employees to the hazard of falling approximately 35 feet to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   On or about October 12, 2022, employees were accessing the roof of a new residential home under construction with a ladder not fully extended at least three feet past the roof line, exposing the employees to a hazard of falling approximately fifteen feet to the ground.
