29 CFR  1910.303(b)(8)(i):Electrical equipment was not firmly secured to the surface on which it was mounted:  a. Whelco Industries, LLC worksite located 28210 Cedar Park Blvd, Perrysburg, Ohio 43551: On or about September 29, 2022, the employer did not ensure a 480V "motor test" electrical panel was secured as designed to prevent unintended displacement. An employee operating on or around the cabinet in this condition was exposed to crushing hazards.

Whelco Industries, LLC worksite located 28210 Cedar Park Blvd, Perrysburg, Ohio 43551:  On or about October 13, 2022,  The employer did not enter each recordable injury or illness on the OSHA 300 log and 301 incident report within 7 calendar days of receiving information that a recordable injury or illness has occurred.  A recordable Injury occurred on September 29, 2022 and was not recorded.
