On or about October 11, 2022, at the above addressed jobsite, employees performing plumbing activities created holes in the floor and did not cover the holes, thereby exposing other contractors' employees to hazards associated with slips, trips, and falls.

On or about October 11, 2022, at the above addressed jobsite, stairways used to gain access to the upper levels were not equipped with at least one handrail and one stairrail system along each unprotected side or edge, thereby exposing employees to slips, trips, and fall hazards.
