29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about October 14, 2022, employees were exposed to a trench cave in while performing water line repair at a commercial site located at 6031 Royalton Road, North Royalton, Ohio 44133.  An employee was working in a trench that was approximately 9 feet deep and the employer did not ensure that he was using cave in protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about October 14, 2022, employees were exposed to a fall of approximately 9 feet while performing water line repair at a commercial site located at 6031 Royalton Road, North Royalton, Ohio 44133.  An employee was working in a trench that was approximately 9 feet deep; the employee was using an A-frame ladder to access the trench, and the employer did not ensure that it extended three feet above the landing area.

An employee was working in a trench that was approximately 9 feet deep; the employee was using an A-frame ladder to access the trench, and the employer did not ensure that the ladder was being used for purposes as it was designed.
