On or about October 14, 2022, employees were exposed to a trench cave in while performing water line repair at a commercial site located at 6031 Royalton Road, North Royalton, Ohio 44133. An employee was working in a trench that was approximately 9 feet deep and the employer did not ensure that he was using cave in protection.

An employee was working in a trench that was approximately 9 feet deep; the employee was using an A-frame ladder to access the trench, and the employer did not ensure that it extended three feet above the landing area.
