29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At a worksite located at 3526 SE Corbin Dr, Greenwood MO, an employee was exposed to electrocution and electrical shock hazards.  The employee was exposed to these hazards when the employer allowed him to use an electrical extension cord that had damage related to strain relief.

29 CFR  1926.501(b)(13):Each employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a worksite located at 3526 SE Corbin Dr, Greenwood MO, employees were exposed to hazards from falls greater than six feet.  The employer exposed the employees to these hazards by allowing them to work at heights greater than six feet without the use of appropriate fall protection or fall mitigation measures.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 3526 SE Corbin Dr, Greenwood MO, an employee was exposed to fall hazards.  The employee was exposed to these hazards when the employer allowed him to use a ladder that did not extend at least three feet above the landing for which it was used to gain access.
