29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:  On or about October 17, 2022, employees were exposed to a fall hazard up to 20 feet while installing roofing material on a residential roof and a suitable fall protection system was not used.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about October 17, 2022, employees were exposed to a fall hazard of approximately 20 feet while ascending and descending an extension ladder to access the second story roof because the side rails did not extend at least three feet above the landing surface.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:  On or about October 17, 2022, the employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employee's name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
