29 CFR  1926.501(b)(4)(i): Each employee on a working/walking surface was not protected from falling through holes more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) Jobsite located at or near 703 Chrome Rd, Oxford, PA - On or about October 12, 2022, the employer did not ensure an employee exposed to falls of approximately 14 feet, while tasked to cut a floor hole on the second story of a commercial building was protected from falling through holes by personal fall arrest systems, covers, or guardrail systems erected around such holes. The employee fell through the hole sustaining injuries.
