OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flash fire/explosion hazards while using a vacuum truck to collect a combustible metal dust, composed of steel and titanium, from the material discharge bin of an outdoor dust collector.   On or about October 18, 2022, three employees utilized a vacuum truck to remove combustible metal abrasive blasting dust containing steel and titanium from a steel bin beneath a dust collector. The use of a vacuum truck for this purpose led to a combustible dust flash fire at the dust collection bin (through the generation of a dense, localized dust cloud and exposure to a source of ignition) exposing employees to thermal burn hazards.
