29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) On or about October 24, 2022, and at times prior, two employees were exposed to fall hazards of approximately 14 feet and greater while engaged in residential stucco restoration on an apartment building. The employees did not use a fall restraint system to prevent falls to a lower level while working in the basket of an aerial lift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) On or about October 24, 2022, and at times prior, two employees were exposed to fall hazards of approximately 14 feet and greater while working inside the basket of an aerial boom lift. The employees did not have the knowledge and understanding of fall hazards using the boom lift and were not provided with scaffold training.
