29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job site, materials and equipment to be made by competent persons designated by the employer.  On or about October 17, 2022, and at times prior thereto, the employer did not perform frequent and regular inspections of the job site, exposing workers to hazards such as, but not limited to struck-by, trips and falls.

29 CFR 1926.501(b)(4)(ii): Each worker on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers.  On or about October 17, 2022, workers were walking around, sweeping and shoveling dirt around open pier holes without the holes being covered, exposing workers to trip and fall hazards.
