Yonge Street, Ormond Beach FL: On or about October 27, 2022, in the warehouse stock area, the employer exposed workers to struck-by and collapse hazards, in that the employer continued use of furniture storage bays of the custom-built wooden racking system where stanchion and framing had been damaged from forklift strikes. b. At 685 S. Yonge Street, Ormond Beach FL: On or about October 27, 2022, in the warehouse stock area, the employer exposed employees to struck-by and crushing hazards in that the the custom built wooden racking system had not been evaluated for load capacity or protected from damage from forklift traffic. c. At 685 S. Yonge Street, Ormond Beach FL: On or about October 27, 2022, above "The Shop" furniture repair area, the employer exposed employees working in the repair area to crushing and struck-by hazards in that the furniture and pallet storage mezzanine above "The Shop" did not have a load rating.
