29 CFR  1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) 18 N. Caesar Rodney Avenue, Camden-Wyoming, DE 19934: On or about 10/27/2022, employees were working working inside of a trench/excavation adding a new sewer connection to the existing infrastructure and the  employer did not ensure a ladder or other safe means of egress was located inside the trench/excavation that was 4 feet deep, 6 feet wide and more than 15 feet long.

29 CFR  1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, such as in excavations in landfill areas or excavations in areas where hazardous gases may exist:  a) 18 N. Caesar Rodney Avenue, Camden-Wyoming, DE 19934: On or about 10/27/2022, employees worked inside a trench/excavation that was not inspected by a competent person prior to employees entering. Employees engaged in excavation activities while installing a sewer pipe connection to existing sewer infrastructure where no atmospheric testing was completed prior to entry into a trench approximately 4 feet deep and 6 feet wide
