29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:  Employees engaged in residential roofing activities at 8005 North 167th Avenue, Bennington, Nebraska on or about October 27, 2022, were exposed to fall hazards in that the employees were not using fall protection when exposed to a fall of approximately 10 feet from the ground while working on a roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees engaged in residential roofing activities at 8005 North 167th Avenue, Bennington, Nebraska on or about October 27, 2022, were exposed to fall hazards in that the employees were using a ladder that was not extended at least 3 feet above the roof level that was being accessed.
