29 CFR  1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely.  a) At Serena & Lily, Inc. located at 2016 Trade Center Boulevard, Rincon, GA, 31326: on or about October 28, 2022, and at times prior to, the employer exposed employees to struck by hazards in that employees were not retrained on Powered Industrial Truck (PITs) operations after being observed operating unsafely and/or being involved in a near-miss or accident involving the PITs.
