29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Jobsite located at 8334 Redbird St. Lenexa Kansas, employees engaged in residential roofing activities from a 7:12 pitch roof were exposed to fall hazards of approximately 13 feet from the eave of the residence to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.
