OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to stuck by / caught between hazards.     a) Dewport Road, Baldwinsville, NY 13027, on or about 11/01/22:  Workers were exposed to struck-by / caught-between hazards when unloading formwork from a shop-built, non approved, form basket.  A worker was crushed after releasing the restraint around the formwork and the material  tipped over and fell onto him.  The injured employee suffered a T-7 fracture (spine), and 2 collapsed lungs and was transported to Upstate hospital.
