The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  (a) Residential roof structure, various locations; Workers engaged in roofing construction activities from a steep-slope roof at approximate heights ranging between 9-feet, 10-inches to 14-feet, 4-inches above ground level were attached to rope lifelines that were connected to the front of climbing-type body belts at waist-level.
