On or about November 2, 2022 and at times prior; The employer failed to ensure employees installing trusses on the right side of the home were protected from falls of 18 feet  to the ground due to no fall protection being used.

On or about November 2, 2022 and at times prior; the employer failed to ensure employees installing trusses was trained on how to use their personal fall arrest system and the hazards associated with falls.
