29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at 17033 Magnolia St, Overland Park, KS 66221 employee conducting residential framing activities was exposed to fall hazards while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the structure:  1) One aluminum extension portable ladder at the front part of the residential house did not extend at least 3 feet above the upper landing. Approximate height was 20 feet. 2) Two portable aluminum extension ladder at the right side part of the residential house did not extend at least 3 feet above the upper landing. Approximate height was 11 feet.
