29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) On or about November 3, 2022, employees conducting re-roofing work on a residential building were exposed to a fall hazard of approximately 14 feet when working from a ladder jack scaffold without fall protection.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  On or about November 3, 2022, employees conducting re-roofing of a  residential building roof, did not use fall protection. This exposed  employees to a fall hazard of approximately 14 feet and 10.2 feet from the roof.
