The employer did not train each employee in at least the following topics listed in 1910.30(a)(3)(i)-(iv):  (a) Throughout the facility; Employees utilizing personal fall arrest systems when inspecting grain handling equipment were not trained, prior to use, on the required elements of recognizing and minimizing fall hazards; procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems; and proper hook-up, anchoring, and tie-off techniques; to include equipment inspection and storage.
