29 CFR  1926.502(d)(15):Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  (a)Cazu Framing LLC at 2110 North Main Street, Tea, SD 57064: On or about and at times prior to November 4, 2022 the employer did not ensure that employees were protected from fall hazards by use of appropriate anchorage points when using fall protection systems. A Checkmate FABX self-retracting lanyards was secured around blocking and the top cord of a secured truss using a shock absorbing lanyard. This condition exposed employees to fall hazards greater than six feet.  (b)Cazu Framing LLC at 2110 North Main Street, Tea, SD 57064: On or about and at times prior to November 4, 2022 the employer did not ensure that employees were protected from fall hazards by use of appropriate anchorage points when using fall protection systems. A Checkmate FABX self-retracting lanyards was secured around bracing, secured with a knotted strap. This condition exposed employees to fall hazards greater than six feet.

This condition exposed employees to hazards related to working from heights and falls.

29 CFR  1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a)Cazu Framing LLC at 2110 North Main Street, Tea, SD 57064: On or about and at times prior to November 7, 2022 the employer did not ensure that employees were protected from fall hazards when using a rope-grab fall protection system near the North side of the building. The configuration and manner in which the rope grab system was used created a swing fall hazard that would not prevent employees from falling more than six feet nor contacting any lower level. This condition exposed employees to fall hazards greater than six feet.   (b)Cazu Framing LLC at 2110 North Main Street, Tea, SD 57064: On or about and at times prior to November 7, 2022 the employer did not ensure that employees were protected from fall hazards when using a Checkmate FABX self-retracting lanyard fall protection system near the North side of the building. The configuration and manner in which the Checkmate FABX self-retracting lanyard was used created a swing fall hazard that would not prevent employees from falling more than 6 feet nor contacting any lower level. This condition exposed employees to fall hazards greater than six feet.

This condition exposed employees to fall hazards greater than six feet.
