29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed.  a) Fabrication Area: On or about November 7, 2022, and at times prior, the employer exposed employees to fall hazards, in that self-supporting, A-frame ladders (step ladders) were used as non self-supporting ladders by employees who leaned the folded ladders  against the exterior sides of water tanks during tank assembly to gain access to the tops of the tanks at heights exceeding 10 feet above the lower level.

29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface.  a) Fabrication Area: On or about November 7, 2022, and at times prior, the employer exposed employees to fall hazards, in that the side rails of self-supporting, A-frame ladders (step ladders) did not extend at least 3 feet above the surface accessed when used by employees as non-self-supporting ladders to gain access to the tops of water tanks at heights exceeding 10 feet above the lower level while the employees performed tasks associated with tank assembly.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   a) Fabrication Area, North Wall: On or about November 7, 2022, and at times prior, the employer exposed employees to electrical  hazards, in that a 4-inch by 4-inch metal electrical box containing a 220-volt  receptacle was not mounted to the building structure where employees used the receptacle to provided power to the "FMB" Titan model horizontal band saw (serial number R00377).
