On or about November 8, 2022, St. Louis Powersports, located at 1729 Chase Drive in Fenton, Missouri,  did not remove from service a powered industrial truck in need of repair, defective, or in any way unsafe:  Propane-powered Toyota forklift, Model 8FGCU30, Serial 10958. The forklift was defective at the time of the inspection. The seat had been replaced with a mower seat, with no seatbelt. The brake pedal was missing a brake pad, and bare metal was exposed. The propane tank was improperly installed, with the LPG locator/index pin not engaged.
