29 CFR  1910.23(c)(8): The employer did not ensure that the top cap and the top step of a step ladder were not used as steps.  Trinity College, Student Dormitory area: On or about October 31, 2022, an employee was exposed to falls from a four-foot fiberglass stepladder when the employee stood on the cap of the ladder to access an area above a first-floor window of a dormitory.
