29 CFR  1910.1030(f)(3)(ii)(A):The post-exposure evaluation and follow-up of an exposure incident did not include testing the source individual's blood as soon as feasible and after consent was obtained in order to determine HBV and HIV infectivity:  a) Operating Room #5:   On or about 7/11/22, following an exposure incident the employer did not attempt to obtain consent of the source individual and test the source individual's blood to determine HBV and HIV infectivity  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19, failure to comply will result in additional penalty of $ 1,000.00.
