In a 400S JLG aerial lift, located at 7490 Gate Parkway, Jacksonville, FL: On or about November 14, 2022, the employer exposed employees to a 12-foot fall hazard, in that fall protection was not utilized while employees were conducting ironwork operations for a commercial building.

In a 400S JLG aerial lift, located at 7490 Gate Parkway, Jacksonville, FL: On or about November 14, 2022, the employer exposed employees to a 12-foot fall hazard, in that an employee was standing on the rails of an aerial lift while conducting ironwork operations for a commercial building.
