29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On November 14, 2022, at the above addressed jobsite, a portable ladder was used by employees to access and egress the roof, measuring approximately 11 feet four (4) inches from ground to eve, was not extended to have the side rails extend at least three (3) feet above the roofline.    No abatement certification or documentation is required for this item.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  On November 14, 2022, at the above addressed jobsite, employees engaged in roofing activities were exposed to falls of greater than 11 feet without the use of fall protection or a warning line and safety monitoring system.  No abatement certification or documentation is required for this item.  Black Square Roofing & Exterior, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(10), which was contained in OSHA inspection number 1291587, citation number 2, item number 1 and was affirmed as a final order on June 4, 2018, with respect to a workplace located at 805 Grove Drive in Buffalo Grove, Illinois.  Black Square Roofing & Exterior, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(10), which was contained in OSHA inspection number 1295580, citation number 1, item number 1 and was affirmed as a final order on October 15, 2018, with respect to a workplace located at 805 Grove Drive in Buffalo Grove, Illinois.
