29 CFR  1926.502(b)(2)(iv):Other structural members (such as additional midrails and architectural panels) shall be installed such that there are no openings in the guardrail system that are more than 19 inches (.5 m) wide.  On or about November 7, 2022 at the construction site, subcontractors were working at the edge of an excavation pit 6 feet (1.8 m) or more in depth were not protected from falling into the excavation pit due to openings in the guardrail system more than 19 inches (.5 m) wide.
