29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  Location - 9 Pelham Road, Lexington, MA  On or about 11/14/2022, an employee fell eleven feet to the ground below while working from a metal residential roof with no means of fall protection.
