29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 31, 2022, and at times prior thereto, employees engaged in applying roofing materials were not provided fall protection exposing employees to an unprotected fall hazard of approximately 20 feet to the ground below.

29 CFR  1926.852(a):  Material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected:  On or about October 31, 2022, and at times prior thereto, the employer did not ensure roofing materials were not thrown down to lower levels without debris chutes and controlled access zones exposing contract workers below to struck-by hazards due to materials being thrown down 3 stories.
