29 CFR  1910.147(c)(6)(ii):  The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.   An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified.  Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.178(l)(4)(i): The powered industrial truck (PITs) operators were not trained and evaluated at least once every three years:  At worksite located at 400 Donaldson Rd, Chillicothe, Missouri.  Employees were exposed to struck by hazards while working in the warehouse and loading dock areas. The employer failed to ensure that the powered industrial truck operators for the facility  obtained recertification every three (3) years per the instructions of the standard.

Employees in the production, shipping receiving area were exposed to a serious struck by hazard from the Forklift that was in operation, without the rubber cover on the brake pedal. Including but not limited to the Clark Forklift Serial number TMX-250-0240-9977.

Storage items and permanently mounted fork lift battery chargers were blocking free access to the electrical box. Including but not limited to battery charger 2.
