29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a): On or about November 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the RG-125 Amada Press Brake. Employees were exposed to crushing and amputation hazards. (b): On or about November 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the RG-80 Amada Press Brake. Employees were exposed to crushing or amputation hazards. (c): On or about November 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the RG-50 Amada Press Brake. Employees were exposed to crushing or amputation hazards. (d): On or about November 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the RG-50 Amada Press Brake. Employees were exposed to crushing or amputation hazards. (e): On or about December 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the Di-Acro/Houdaille Hydra-Power Press Brake Model 14-481. Employees were exposed to crushing or amputation hazards. (f): On or about December 21, 2022, the employer did not to ensure that point of operation guarding was installed to prevent employees from having any part of their body in the danger zone during operation of the Haeger 824 Plus Fastener Insertion System. Employees were exposed to crushing or amputation hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

All press welding machine operations, where there is a possibility of the operator's fingers being under the point of operation, shall be effectively guarded by the use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation, 1910.217. All chains, gears, operating bus linkage, and belts shall be protected by adequate guards, in accordance with 1910.219 of this part. (a): On or about December 21, 2022, the employer did not ensure that press welding machine operations were effectively guarded during operation of the Stanco LORS AR50-36 Spot Welder. Employees were exposed to burns and amputation hazards. (b): On or about December 21, 2022, the employer did not ensure that press welding machine operations were effectively guarded during operation of the Stanco LORS AR50-36 Spot Welder. Employees were exposed to burns and amputation hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

(a): On or about November 21, 2022, Electrical equipment was not installed and used in accordance with instructions included in the listing or labeling. The employer mounted Relocatable Power Taps (RPTs) to a storage rack support beam above a welding table in order to provide electricity to a series of fans and associated electronics, exposing employees to fire hazards.

29 CFR 1910.134(c)(2)(i):An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); (a): On or about November 21, 2022, the employer did not provide respirator users with the information contained in Appendix D of the respiratory protection standard, 29 CFR 1910.134. Employees were observed wearing N95 masks while welding. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
