Burkholder Construction, LTD. worksite located 8105 Malone Rd, Shiloh, Ohio, 44878: On or about November 16, 2022, the employer did not provide training and evaluation to ensure that employees, operating powered industrial vehicles, such as Sky Trak Model # 6036 rough terrain forklift, was competent to safely complete the task. Employees were exposed to struck-by, and caught in or between hazards.

The employees performing this work were exposed to falls of approximately 27 feet to the ground below.
