29 CFR  1910.23(b)(10):  The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.   a) On or about November 16, 2022, employees were exposed to fall hazards during maintenance operations at 14400 Howe Road, Strongsville, Ohio 44136; the employer did not ensure that any ladder with structural or other defects were immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced, exposing employees to a fall hazard while using the 28ft WERNER (silver) aluminum extension ladder. The feet of the ladder were not functional.

29 CFR  1910.23(b)(13):The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder.     a) On or about September 16, 2022, employees were exposed to fall hazards during pressure washing operations at 14400 Howe Road, Strongsville, Ohio 44136; the employer did not ensure that employees did not carry any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder exposing the employee to a fall hazard. Employees climb with and operate a pressure washer requiring the use of both hands and are not able to maintain three points of contact.
