29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced. a) On or about November 16, 2022, employees were exposed to fall hazards during maintenance operations at 14400 Howe Road, Strongsville, Ohio 44136; the employer did not ensure that any ladder with structural or other defects were immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced, exposing employees to a fall hazard while using the 28ft WERNER (silver) aluminum extension ladder. The feet of the ladder were not functional.

Employees climb with and operate a pressure washer requiring the use of both hands and are not able to maintain three points of contact.
