29 CFR  1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  On or about July 20, 2022, the employer did not ensure that case number 4A2207P9L940001 on the employer's OSHA 300 form for 2022 contained a complete description of  the injury or illness, parts of body affected, and object/substance that directly injured or made person ill.

29 CFR  1910.22(a)(3):(3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  On the campus located at 2420 Nicolet Drive, Green Bay, WI 54311, in the University Union's Kitchen, employees were walking or working on loose metal grating in front of the ovens, exposing employees to slip and fall hazards.
