(a) On or about, November 9, 2022, The employer failed to fully document its training of employees regarding the employer's policy found in its emergency response plan that the use of fire extinguishers to fight fires is strictly limited to minor, incipient stage fires, and all fires beyond the minor, incipient stage require evacuation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.156(c)(1):The employer shall provide training and education for all fire brigade members commensurate with those duties and functions that fire brigade members are expected to perform. Such training and education shall be provided to fire brigade members before they perform fire brigade emergency activities. Fire brigade leaders and training instructors shall be provided with training and education which is more comprehensive than that provided to the general membership of the fire brigade. (a) On or about November 9. 2022, de facto fire brigade members responding to a furnace ventilation stack fire were exposed to hazardous by-products of a fire (smoke, toxic gases, heated air, etc.) and burns from flame, in that they did not receive specific training for the emergency operations in which they were to participate, training on their specific roles or duties, or training on the use of personal protective equipment that may be required. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.156(f)(1)(ii): Approved self-contained breathing apparatus with full-facepiece, or with approved helmet or hood configuration, shall be provided to and worn by fire brigade members while working inside buildings or confined spaces where toxic products of combustion or an oxygen deficiency may be present. Such apparatus shall also be worn during emergency situations involving toxic substances. (a) On or about November 9, 2022, the employer failed to provide each de facto fire brigade member with an approved self-contained breathing apparatus with full-facepiece, or with approved helmet or hood configuration. As a result, these designated employees were exposed to hazards such as inhalation of hazardous by-products of a fire (smoke, toxic gases, heated air, etc.) when fighting sintering furnace stack fires that developed beyond the incipient stage. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
