29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  Location: 438 Old Bedford Road, Concord, MA  On or about 11/18/22, employees working from a ladder-jack scaffold were exposed to falls posed by a cantilevered portion of the scaffold platform which extended more than 18 inches over its support.

29 CFR  1926.452(m)(1): Crawling boards did not extend from the roof peak to the eaves when used in connection with roof construction, repair, or maintenance.  Location: 438 Old Bedford Road, Concord, MA  On or about 11/18/22, employees were utilizing a crawling board (chicken ladder) on the roof of a new residential construction to install metal roofing. The ladder utilized did not run the full length of the roof from peak to eave.

Location: 438 Old Bedford Road, Concord, MA  On or about 11/18/22, employees were exposed to a fall hazard of 17-feet in residential construction while installing metal roofing without any means of fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Location: 438 Old Bedford Road, Concord, MA  On or about 11/18/22, employees were exposed to falls while climbing and descending a portable ladder that did not extend at least 3 feet above the upper landing (Roof).
