29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top of roof - On or about November 19, 2022, the employer instructed his employees to carry shingles onto the roof of a two-story apartment building and apply them to the roof without the use of a fall protection system exposing them to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Front of building - On or about November 19, 2022, the employer had the employees use the aluminum extension ladder that did not extend 3 feet past the roofs edge to carry roofing shingles up to the roof exposing them to fall hazards.
